On Tuesday 10 September Amy Legg, Clinical Pharmacist, chaired our fourth annual Worldwide Precision Dosing seminar with speakers from the US, Austria, France and Australia. We covered when and how to implement continuous infusions of antimicrobials in the ICU, precision dosing of aminoglycosides and dalbavancin, and practical case examples of using model informed precision dosing in clinical practice.
We were fortunate to host Dr Karlee Johnston from Australian National University, Canberra, who provided an engaging and practical session on how to make changes to practice in the ICU. Dr Julie Anne Justo and Dr Emily Heil presented a masterclass on aminoglycoside PKPD targets and dosing. Precision dosing of the exciting new kid on the block, dalbavancin, was described by Dr Matthieu Gregoire and is a must watch for anyone who is thinking of using this in practice. Finally, all the way from Vienna, Dr Iris Minichmayr explained the role of model informed precision dosing from theory to practice!
Highlights included:
- The why, who and how of continuous infusions in ICU, particularly relevant after the recent publication by UQCCR researchers of the systematic review and meta-analysis of continuous infusions of beta lactams in critically ill patients.
- “Kill the bug dead and let it ride” Dr Julie Anne Justo’s vivid description of how aminoglycosides were thought to work, and the shift from Cmax/MIC to AUC/MIC ratio as the PD parameter of interest for these drugs.
- Do we need the same PD target on day 1 and day 14 for long-acting glycopeptides? Because it's impossible. A new way of thinking about dosing because of dalbavancin’s unique PK profile.
- You can predict the future, just use model informed precision dosing! Dr Iris Minichmayr shows with case examples new technology supporting precision dosing to optimise patient care.
